ABOUT THE FOUNDATION

Black River Education and Wellness Foundation (“the Foundation”) is a tax-exempt 501 (c) (3) charitable foundation organized in 1985. Originally, the Foundation was chartered to support the operations of the then-existing Elyria Memorial Hospital, and was known as the Elyria Memorial Hospital Foundation. Through the year 2013, the Foundation continued that mission. Commencing January 1, 2014, with the acquisition of what is now known as UH Elyria Medical Center by University Hospitals of Cleveland, the Foundation broadened its mission and purpose beyond the support of just the Elyria Medical Center in order to maintain a commitment to the communities long served by Elyria MemoriaL Hospital and to have a meaningful impact upon health issues in those communities.

HOW TO APPLY

Requests for Grant Funding are entertained and considered by the Foundation on a rolling basis throughout the calendar year. Decisions as to the merits of a request, and decisions as to the timing of any disbursement of funding, are entirely at the discretion of the Foundation Board.

Only those requests to fund a project or program which can be demonstrated to be directly related to the health issues of a community, or a group within a community, or which can be demonstrated to be likely to improve health, quality of life, or access to care within a community in Lorain County, will be considered by the Foundation for funding.

STEP 1. COMPLETE THE ATTACHED REQUEST FOR GRANT FUNDING APPLICATION.

It is recommended that requesting organizations submit as much information as possible in support of the request. Of particular importance is information describing the relationship of the proposed or continuing project or program to an impact upon the improved health or improved access to care within a community. If the purchase of equipment is to be a part of the project or program, any information regarding the proposed equipment should be appended to the request application.

STEP 2. SIGN AND DATE THE REQUEST FOR GRANT FUNDING.

Ascertain that the person signing the request application has authority to act on behalf of the organization.

STEP 3. SUBMIT THE REQUEST FOR GRANT FUNDING (ALONG WITH ANY SUPPORTING DOCUMENTATION) VIA MAIL TO:

The person identified as the Primary Contact for the requesting organization will be contacted to acknowledge receipt of the Request by the Foundation. If, at the Foundation Board’s discretion, the Board wishes to interview a representative of the requesting organization regarding any aspect of the Request, or the project or program, the Foundation will contact the Primary Contact of the organization at the number provided.
